Dear Parents/Guardians,
If your child has medication(s) in the nurse’s office at Cinnaminson Middle School, please remember to pick it up. Medication must be dropped off and picked up by the parent/guardian.
When you pick up your child’s medication, feel free to also pick up any relevant Medication Permission Forms for the upcoming school year. For your convenience, these forms are available on the district website, as well as each individual school website or from the nurse’s office. New medication permission forms for each medication are required each school year.
- If your child uses asthma medication, an Asthma Treatment Plan must be completed each school year by their healthcare provider.
- If your child requires diabetic medication, a Diabetes Treatment Plan must be completed each school year by their healthcare provider.
- If your child requires emergency medication for life-threatening allergic reactions such as Epinephrine (Epi-pen) and/or an antihistamine (Benadryl/Zyrtec), then a FARE Plan must be completed each school year by their healthcare provider.
- If your child requires emergency medication for seizures, then a Seizure Action Plan must be completed each school year by their healthcare provider.
The last day for medication pick up is 6/18/2026 by 12:00PM. Please make arrangements to pick up your child’s medication as no medication will be stored over the summer and will be picked up by Medical Waste. If you have any questions or concerns, please call the nurse’s office.
